Billing and Insurance Policy

In the spirit of cooperation, we are always happy to discuss fully and openly our fees and payment policy. We appreciate your choosing The Hand Care Center/Shoulder and Elbow Institute for our medical care and will work with you in making arrangements that are mutually satisfactory. Please note the following regarding our financial policy:

We will need to copy your insurance card for billing purposes. In addition to billing information, it contains information needed to obtain certification for hospital admission in case of surgery. We also ask that you provide us with a completed, signed form that includes the assignment of benefits.

Examination fees are to be paid at the time of the visit.
If you are covered under a Preferred Health Plan (such as Blue Cross Prudent Buyer, Blue Shield, Pacificare etc.), your insurance company will be billed directly.

If your plan calls for a co-pay, we expect and appreciate payment at the time of the visit.

If your plan requires prior authorization from your primary care physician, please obtain this authorization before your visit to The Hand Care Center/Shoulder and Elbow Institute.

We are contracted with most major plans.

As Participating Physicians we will bill Medicare for your treatment or surgery, and also bill your secondary insurance.

You will receive a statement once your insurance has paid. We also ask the payments to be made on our portion due. Your policy will tell you whether you have a copay.

We wish to stress that the financial responsibility for services rendered rests with you or your family regardless of any insurance coverage. Your insurance policy is a contract between your insurance company and you.
